 SEPTIC SYSTEM CHECKLIST FOR APPLICATION

Inspection required before backfilling; keep project info on-site. Refer to Utah Code R317-4.

FOR SUBMITTAL

Three copies of a legible plot plan with system design and trench details.
All soil analysis documentation.
Outline and floor plan of all plumbed buildings.
Proof of water service or private water rights with a protection zone.
Any specialty documentation required by the local municipality.

FOR SYSTEM DESIGNERS

Conducted by an individual certified by the Utah Division of Water Quality.
Soil conditions from exploration to 10 ft or refusal; excavations over 6 ft require exploration 4 ft below proposed seepage depth.
Unsigned certificates are not accepted.
USDA soil descriptions only; USCS soil types are not accepted under R317-4.
Stabilized percolation test results, if required by Table 6 of R317-4, for the proposed absorption area.
Construction lot, block, and site address (provide directions in remote areas).
Lot size, dimensions, and direction of north.
Location and dimensions of roads, driveways, parking areas, and paved roads.
Ground surface contours, relative elevations, or statement of grade.
Wells, springs, protection zones, ditches, ponds, lakes, or streams on or within 100 ft of the property.
All water service lines on the property.
Scaled/dimensioned system plan showing required components and setbacks.
Septic tank size and manufacturer.
Location of water source for the dwelling and the distance from the septic system.
Location of all soil exploration pits and percolation test holes.
Seepage trench cross-section showing excavation, gravel fill, drain line, and backfill barrier.
Gravel-less chamber systems: specify manufacturer, model, and type (A or B).

FOR PROPERTY OWNERS

Public water connections: provide utility documentation confirming service.
Non-public connections: submit water rights, quality, septic setback location, and required pressure/flow verification.
Water Rights & Protection: Proof of owner water rights and an established protection zone.
Well Log: Submission of the official well log via waterrights.utah.gov.
Water Quality Testing: Submission of satisfactory chemical and bacteriological analyses.

COUNTY-SPECIFIC REQUIREMENTS

Washington County permits require written approval from the applicable wastewater authority. Approved entities include:

Iron County: Submit Division of Wildlife Resources prairie dog clearance with the permit application.
Kane & Garfield Counties: Clearance may also be required; applicants should verify local habitat rules before submission.
