BIRTH & DEATH CERTIFICATES
Certified copies of birth and death certificates may be requested through any of our offices for events that occurred in Utah. We also have limited access to marriage and divorce records and can refer you to the appropriate office when we are unable to assist.
If the event occurred outside of Utah, you will need to contact the vital records office in that jurisdiction, as each state maintains its own records. The national directory of vital records offices provides contact information for each agency.
HOW TO REQUEST A CERTIFICATE
There are two ways to request a certificate: online or in-person.
Online
Request through the Utah Vital Records website. Orders typically arrive by mail within 3 weeks (processing times may vary).
In-Person
Visit one of our offices. Many requests can be completed the same day, depending on eligibility and required documentation. Refer to the sections below for requirements by certificate type.
BIRTH CERTIFICATE — FOR THE APPLICANT
Required Identification
Valid government-issued photo identification. Refer to the ID Requirements section for acceptable documents.

- Date of birth
- Place of birth
- Parent(s) full names (including mother’s maiden name if known)
- Contact information (name, phone, address, email)

- $22 per certificate
- $10 per additional copy
Additional fees may apply for amendments, corrections, or reissuance. A $0.75 card processing fee applies.
BIRTH CERTIFICATE — FOR SOMEONE ELSE
Required Identification
Valid government-issued photo identification. Refer to the ID Requirements section for acceptable documents.
Eligibility Requirements
You must have a legal or qualifying relationship to the individual listed on the birth certificate.

- Sibling
- Grandparent or grandchild
- Legal guardian

FREQUENTLY ASKED QUESTIONS
Q: Can I get a certificate if I was born in a county outside of Southwest Utah?
A: Yes. We issue certified copies for all Utah birth records.
Q: How do I know if my copy is certified?
A: Certified copies have a seal and are printed on security paper.
Q: How do I get a birth certificate for a home birth?
A: If a midwife assisted, the midwife files the birth record. If not, contact the Utah Office of Vital Records. After registration, you may request a certified copy.
Q: When do I need an apostille?
A: For documents used in another country (employment, marriage, adoption, or missionary service). Visit Utah Authentications for more information.
Q: How do I add a father to a birth certificate?
A: Both parents need to complete a Voluntary Declaration of Paternity.
