The Southwest Utah Public Health Department (SWUPHD) respects the privacy of every individual who uses our services, including our website. This Privacy Statement outlines the information SWUPHD may collect and how that information may be used, both online and through peronal forms. We are committed to protecting the privacy and accuracy of confidential information to the extent possible, subject to provisions of state and federal law. We do not re-distribute or sell personal information and data that we collect.

RESPONSIBILITY FOR THIS SITE

The SWUPHD is responsible for this website. If you have any questions or concerns, or if you would like to request access, corrections, amendments, or removal your personal data, please contact us. To file a complaint with the State’s data privacy ombudsperson, click here. An at-risk employee may request that the at-risk employee’s personal information be classified as a private record under Section 63G-2-302 by contacting Human Resources.

PERSONALLY IDENTIFIABLE INFORMATION

When you visit the SWUPHD website (swuhealth.gov) or our facilities, we won’t collect any personally identifiable information about you (such as name, address, telephone number, property information, or email address) unless you provide it to us voluntarily (such as completing a form online or on paper).

Should you wish not to provide any personally identifiable information, you may utilize the services by using the appropriate non-electronic means or you may decline to pursue the services.

In Utah, access to personally identifiable information in public records at state and local levels of government in Utah is controlled primarily by Chapter 2, Title 63 of the Utah Code, entitled the Government Records Access and Management Act (GRAMA). Personal information provided to SWUPHD online or in-person, including personal data, that would be classified as a “public record” under Section 63G-2-301 of this statute may be available to the public as provided by Section Section 63G-2-201. Other laws, including federal laws or other Utah statutes, also address confidentiality issues and are followed.

SWUPHD DOES NOT sell your personal data or share your personal data with private companies. Washington County will not give your personally identifiable information to anyone outside SWUPHD unless required by law.

Email or other information requests sent to SWUPHD via its website or offices may be maintained in order to respond to the request, forward that request to the appropriate agency, and/or communicate updates.

SWUPHD does not attempt to gain personally identifiable information on individual website users or associate individual users and with their IP (Internet Protocol) numbers. SWUPHD does track the IP numbers of computers used to visit the site as part of our statistical analysis on the use of our website for website design and accessibility improvement.

This Privacy Statement was last revised on October 24, 2025. We may change this Privacy Statement at any time and for any reason.

We encourage you to review this Privacy Statement each time you visit the website. If we make a significant change to our Privacy Statement, we’ll post a notice on the homepage of our website for a period of time after the change is made.

RESPONSIBILITY FOR EXTERNAL SITES

The SWUPHD website may link to various non-SWUPHD websites. Many of these external sites, including private sector sites, are not subject to Chapter 2 of the Utah Code and may also not be subject to other sections of the Utah Code or federal law. Visitors to those sites are advised to check the privacy statements of those sites and be cautious about providing personally identifiable information without a clear understanding of how the information will be used

SWUPHD is committed to keeping the data you provide us with secure and will take reasonable precautions to protect your personally identifiable information from loss, misuse, or alteration. SWUPHD employs industry standards in securing this information.

USDA NON-DISCRIMINATION AND EQUAL ACCESS POLICY

In accordance with Federal civil rights law and U.S. Department of Agriculture (USDA) civil rights regulations and policies, the USDA, its agencies, offices, employees, and institutions participating in or administering USDA programs are prohibited from discriminating based on race, color, national origin, sex, disability, age, or reprisal or retaliation for prior civil rights activity in any program or activity conducted or funded by USDA. Persons with disabilities who require alternative means of communication for information (e.g., Braille, large print, audiotape, American Sign Language, etc.), should contact the Agency (state or local) where they applied for benefits. Individuals who are deaf, hard of hearing, or have speech disabilities may contact USDA through the Federal Relay Service at (800) 877-8339. Additionally, information may be made available in languages other than English.

To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, (AD-3027) found online at http://www.ocio.usda.gov/sites/default/files/docs/2012/Complain_combined_6_8_12.pdf or at any USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form.
