This Privacy Statement describes the information SWUPHD may collect and how it may be used, both online and through personal forms. We are committed to protecting the privacy and accuracy of confidential information to the fullest extent possible, in accordance with applicable state and federal laws. SWUPHD does not sell or share the personal information we collect.

PERSONALLY IDENTIFIABLE INFORMATION
SWUPHD will not collect personally identifiable information unless you provide it voluntarily (for example, by completing a form online or on paper). You may still access services using non-electronic methods if you choose not to provide personal information.
In Utah, access to personally identifiable information in public records is governed by the Government Records Access and Management Act (GRAMA), Chapter 2, Title 63 of the Utah Code. Personal information provided to SWUPHD that is classified as a “public record” under Section 63G-2-301 may be available to the public as outlined in Section 63G-2-201. SWUPHD follows all applicable state and federal laws regarding confidentiality.
SWUPHD does not sell your personal data or share it with private companies. Washington County will not release personally identifiable information outside SWUPHD unless required by law.
Information requests sent to SWUPHD may be maintained to respond to the request, forward it to the appropriate agency, or provide updates. SWUPHD does not attempt to associate individual users with their IP addresses, though IPs may be tracked anonymously for statistical analysis to improve website design and accessibility.
RESPONSIBILITY FOR EXTERNAL SITES
The SWUPHD website may link to external websites, including private sector sites, that are not subject to the Government Records Access and Management Act (GRAMA), Chapter 2, Title 63 of the Utah Code, or other Utah or federal privacy laws. USDA NON-DISCRIMINATION AND EQUAL ACCESS POLICY
In accordance with federal civil rights law and U.S. Department of Agriculture (USDA) regulations, USDA programs cannot discriminate based on race, color, national origin, sex, disability, age, or reprisal.
