IMMUNIZATIONS

Our clinics provide immunizations for vaccine-preventable diseases across all ages, from children to adults. Vaccines help protect against serious illnesses by training the immune system to recognize and fight harmful germs. Protection typically develops within a few weeks after vaccination, and many vaccines provide long-lasting immunity. Some vaccines, such as those for tetanus and influenza, require periodic boosters to maintain protection.

  • Valid/current photo identification
  • Insurance card (if applicable) – refer to Accepted Insurance Plans section below
  • Payment for any out-of-pocket costs, if needed
  • Wear a short-sleeve shirt or clothing that allows easy access to the upper arm

Intake Form (Complete at Check-In)

Please be prepared to provide the following information:

  • Basic patient information (name, date of birth, age, gender)
  • Contact and address information
  • Emergency contact information
  • Insurance information (if applicable)
  • General health screening information, including medical history and recent vaccinations – refer to Records & Access section below

Minors must be accompanied by a parent or legal guardian.
Otherwise, contact us before your visit for a permission form.

ACCEPTED INSURANCES AND VACCINE PRICING

We accept most major insurance plans for immunizations. Coverage may vary based on your provider and benefits.

If your child is uninsured, low-cost vaccines may be available through the Vaccines for Children (VFC) program.

TUBERCULOSIS (TB) SKIN TEST

Tuberculosis is a bacterial infection that most often affects the lungs but can also affect other parts of the body. If not treated, it can lead to serious illness.

TB Skin Test

A skin test checks for exposure to the bacteria. A small amount of fluid is placed under the skin on your arm, and you will return in 48–72 hours so the test site can be evaluated.

  • Negative: No infection is likely.
  • Positive: Further testing is needed to determine if infection or disease is present.

Who Should Get Tested

Testing is recommended for those at higher risk, including anyone who has been exposed, has traveled to or lived in areas where it is common, or works or lives in high-risk settings.

Cost

Skin tests are $30. Payment is due at the time of service.
