GRAMA REQUEST
GOVERNMENT RECORDS ACCESS AND MANAGEMENT
Utah’s Government Records Access and Management Act (GRAMA) allows you to request access to public records maintained by a government agency. This law outlines your rights to inspect or obtain copies of government records and the process agencies follow to respond to requests.
Submit a Request
- Visit the Open Records Portal.
- In the Agency Name field, search for “Southwest Utah Public Health Department,” then select the Request Records button.
- Sign in on the Utah ID login page. (If you do not have a Utah ID account, follow the prompts to create one. Creating an account allows you to save your progress and receive updates by email.)
- After your form is submitted, our department will review your request and forward it to the appropriate division for processing.
