GRAMA REQUEST

GOVERNMENT RECORDS ACCESS AND MANAGEMENT

Utah’s Government Records Access and Management Act (GRAMA) allows you to request access to public records maintained by a government agency. This law outlines your rights to inspect or obtain copies of government records and the process agencies follow to respond to requests.

Submit a Request

  1. Visit the Open Records Portal.
  2. In the Agency Name field, search for “Southwest Utah Public Health Department,” then select the Request Records button.
  3. Sign in on the Utah ID login page. (If you do not have a Utah ID account, follow the prompts to create one. Creating an account allows you to save your progress and receive updates by email.)
  4. After your form is submitted, our department will review your request and forward it to the appropriate division for processing.
