Helping achieve health equity in our communities. Health equity means every person has access to resources that allow them to reach their full health potential and eliminate health disparities caused by varying life circumstances. Often it can be difficult to find and access resources that can help us achieve health equity, which is why we have community health workers that can help connect you to resources and partners in the community that will achieve this goal. Resources that we can connect you with include: Aging Health Concerns, Children & Youth Support/Protection, Clothing Needs, Disability Services, Domestic Violence Resources, Education, Housing, Employment, Food, Medical/Dental Care, Utilities, Veteran Services, and more.
